Job summary

The incumbent provides advanced professional nursing case management and care coordination services to eligible American Indian beneficiaries, independently managing complex case management situations in the Emergency Department at the Northern Navajo Medical Center, Shiprock, NM. Qualifications

• Requires a professional nursing degree from an accredited program recognized by appropriate U.S. accrediting bodies.
• National Council Licensure Examination (NCLEX) is required for licensure as a registered nurse.
• Current, active, full, unrestricted nursing license in the United States, District of Columbia, Puerto Rico, or U.S. territories.

• Manage a caseload of patients with chronic conditions by assessing needs and creating personalized care plans.
