Qualifications

• Graduate of an ACPE-accredited School of Pharmacy with a Doctor of Pharmacy Degree (Pharm.D.).
• Licensed in the State of Arizona or eligible within 90 days of hire.

Responsibilities

• Provide clinical pharmacy services for patients and act as a resource and leader to physicians and caregivers.
• Coordinate pharmacokinetics, parenteral nutrition, and pharmaceutical care across patient care areas.
• Identify and resolve medication-related problems via collaborative practice and interdisciplinary rounding; monitor drug therapy and lab results.
• Assist in education programs; mentor residents in the residency training program.
