The Advanced Clinical Pharmacy Specialist in Emergency Medicine will be responsible for providing a full range of pharmaceutical care services to the specialized patient population within the Emergency Department. The pharmacist will support the Emergency Medicine service during direct patient care and provide onsite assistance with medication reconciliation, emergency response, drug information, and triage care. The pharmacist will serve as a subject matter expert for Emergency Medicine for Jefferson Health New Jersey and will share committee appointments and responsibilities with other Emergency Medicine pharmacy specialists. The pharmacist will also serve as a preceptor for our PGY1 Pharmacy Residency program and pharmacy students.Job Description
Essential Functions
Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
Assures the appropriateness of drug therapy and compliance with hospital policies
Assures implementation of processes to support delivery of quality Pharmacy services
Serves as liaison between the pharmacy department and the Emergency Medicine service
Trains staff as assigned in areas of expertise
Serves on hospital committees including but not limited to Pharmacy and Therapeutics, Emergency Medicine Service Line, regulatory readiness committees
Serve a preceptor to PGY1 pharmacy residents and pharmacy students as assigned
Work with clinical pharmacy department to develop new protocols, service lines, and initiatives to further departmental goals and objectives
Work with pharmacy operations to streamline drug delivery and safe administration within the Emergency Department
Competencies (Knowledge, Skills, and Abilities Required):
Demonstrates advanced problem identification and resolution skills, an advanced therapeutic database and applies this knowledge in daily practice. Recognized as an authority in specialized Pharmacy practice area by members of the interdisciplinary team.
Demonstrates leadership in formulating and achieving patient care goals and mentoring other members of the Department staff in this role
Provides quality pharmaceutical care in accordance with standards of practice, demonstrating expertise, competency, knowledge and skills necessary to service the patients
Promotes and assures compliance with the hospital formulary and the policies and procedures defined by the P & T Committee
Identifies and reports sentinel/serious events, near misses, medication errors and adverse reactions in the electronic event reporting system
Demonstrates, through interaction with medical staff, the ability to effectively change standards of prescribing formulary alternatives
Minimum Education and Experience Requirements:
Education:
Doctor of Pharmacy (PharmD)
Experience:
PGY1 Pharmacy Residency completion required
PGY2 Emergency Medicine Pharmacy Residency or PGY2 Critical Care Pharmacy Residency preferred
If no residency completion, minimum of 5 years of experience as a clinical pharmacist required
If only PGY1 Pharmacy Residency completed, minimum of 3 years' experience as a clinical pharmacist required
Minimum Certifications, Registration or License Requirements:
NJ Pharmacist License required
Board Certified Emergency Medicine Pharmacist (BCEMP) or Board-Certified Critical Care Pharmacist (BCCCP) or Eligibility for Board of Pharmacy Specialties certification required
