Social Workers serving in a variety of departments and clinics are responsible for responding to referrals from the interdisciplinary team. The social worker will complete psychosocial assessments, develop and implement a plan of care that addresses patients' needs, promotes continuity of care, and facilitates transitions to the next level of care. The social worker will provide education, consultation, and serve as a liaison to the healthcare team regarding the emotional, psychological, social, financial, and developmental aspects of care for patients and families.

Minimum Qualifications:

  • Master of Social Work (MSW) from an accredited School of Social Work.
  • License by the State of Ohio Social Work Board (LSW, LISW, or LISW-S).
  • For employees in UCMC Psychiatric Services, NCI-APS Certification must be obtained within 90 days of hire.
  • Experience: 1-2 years minimum; preferred 3-5 years.

Responsibilities include:

  • Engaging patients and/or families into a helping process, assessing psychosocial and mental health needs, identifying barriers, and enhancing health status.
  • Developing and implementing comprehensive care plans in collaboration with patients, families, and healthcare teams.
  • Providing counseling, crisis management, advocacy, education, and facilitating discharge and referrals.
  • Evaluating care effectiveness, summarizing care plans, and ensuring smooth transitions.
  • Providing education and consultation to healthcare professionals about psychosocial issues.
  • Participating in departmental activities, documentation, quality improvement, and community resource linkage.
