Qualifications

• Master's degree in Social Work required.
• Licensed Social Worker (Hawaii) or Licensed Clinical Social Worker (Hawaii).
• At least 1 year experience in a health care setting.
• Basic Life Support (BLS) certification within 0.5 months of hire.

Responsabilités

• Provide psychosocial assessments and care planning in ED/CDU.
• Coordinate discharge planning and community resource referrals.
• Assist patients and families with coping strategies and support.
• Participate in performance improvement and quality initiatives.
