If you are a Case Manager who enjoys working with complex cases that require strong problem-solving skills, this is an exceptional opportunity to work close to home with a community hospital.

Position Summary:

The Case Manager is responsible and accountable for managing care for an assigned patient population by service line and across the continuum of care. The Case Manager works to achieve daily clinical, quality, and cost outcomes by providing well-coordinated experiences for patients and families through the synchronization of care activities of multiple disciplines and negotiation with third-party payers.

  1. Demonstrate excellent assessment skills and knowledge to develop and implement comprehensive care plans tailored to identified needs.
  2. Coordinate all aspects of the patient's care plan, including home health care/hospice referrals, home infusion services, DME, transportation, etc.
  3. Facilitate communication and coordination among members of the interdisciplinary team, involving the patient in decision-making to minimize service fragmentation.
  4. Advocate for the patient by communicating their preferences and ensuring interventions meet their individualized needs and goals.
  5. Provide information about community resources and coordinate service delivery.
  6. Identify and address client risk factors and obstacles to care.
  7. Educate patients and families regarding symptoms, conditions, and treatments.

Hours: 40 hours/week, Day Shift with minimal weekends and holidays. Flexible hours: 8-4:30, 8:30-5, or 9-5:30.

Education and Qualifications:

  • Massachusetts RN Licensure.
  • Current Basic Life Support (BLS) Certification.
  • Bachelor of Science in Nursing (BSN) preferred.
  • ASN required.
  • Recent inpatient case management experience.
  • Minimum of 3 years case management experience.
