Job summary

LiveWell Homecare Agency, LLC provides comprehensive nursing care in collaboration with the healthcare team, using clinical judgment to diagnose and treat human health problems. The RN will assess, plan, implement, educate, and coordinate patient care including discharge planning and education.

The role requires accurate documentation, adherence to protocols, and strong communication to ensure patient safety and optimal outcomes in a homecare/clinical setting.

Qualifications

• Minimum of nine months RN experience, with at least one year in an emergency department or completion of an internship.
• Graduate of an accredited nursing program.
• RN license, BLS, ACLS, NIH Stroke Scale, and either PALS or ENPC within six months of hire.

Responsibilities

• Assessment: Performs accurate multi-system assessments, identifies deviations, and reassesses patients throughout care.
• Planning: Develops individualized care plans, coordinates care with other departments, and delegates tasks appropriately.
• Implementation: Initiates protocols, executes physicians’ plans, administers medications accurately, and documents care meticulously.
• Evaluation: Assesses patient responses, evaluates care effectiveness, and participates in quality assurance activities.
