Job summary

NALHE in Fremont, California seeks an experienced Registered Nurse II to provide patient-centered care across the nursing process and collaborate with the health care team.

The RN II will lead patient/family education, establish priorities, and coordinate safe, effective care in a fast-paced clinical setting. A commitment to confidentiality and high professional standards is required.

Responsibilities

• Performs assessment/data collection focusing on physiologic, psychosocial, cultural, spiritual and cognitive status; includes nutrition, pain, patient/family education, family involvement and patient advocacy.
• Formulates a goal-directed plan of care.
• Implements care in a thorough, skillful, consistent, and continuous manner.
• Establishes priorities based on patient needs and available resources.
• Identifies patient/family learning needs and acts to meet them.
• Demonstrates sensitivity to patient/family rights, age-related needs, cultural and ethical beliefs.
• Provides/coordinates care for patients/patient populations.
• Establishes effective working relationships with health care team, patients, and families.
• Delegates appropriately and coordinates duties of health care team members.
• Evaluates effectiveness of care and communicates using established lines of authority.
• Applies safety principles and handles emergency patient care per protocols.
• Maintains licensure, certifications and ongoing professional development.
• Performs duties with attention to safety, infection control, and risk management.
• Provides care consistent with EMTALA, Nurse Practice Act, Title 22 and applicable regulations.
• Demonstrates proficiency in emergency response and use of computers for documentation and messaging.
• Teamwork with internal and external departments in care provision.
• Medication administration and management including titration and pediatric medications.
• Demonstrates flexible staffing and willingness to float as needed.
