Job summary

Westchester Medical Center Health Network seeks a Mental Health Evaluator to perform psychiatric emergency evaluations, consultations, and crisis interventions in the hospital Emergency Department. You will coordinate aftercare, collaborate with interdisciplinary teams, and ensure timely documentation in the EMR.

The role requires a Master’s in Social Work or Mental Health Counseling and a NY license, with at least two years of relevant post-graduate experience.

Qualifications

• Two years post-graduate experience in a medical, mental health, or human services setting as part of a multidisciplinary team.
• Master’s Degree in Social Work or Mental Health Counseling from an accredited program.
• New York State Social Work or Mental Health Counseling license required.

Responsibilities

• Gather psychosocial and diagnostic information to conduct comprehensive psychiatric evaluations.
• Conduct interviews with patients and collateral resources.
• Collaborate with ED and Psychiatry teams to determine level of care.
• Demonstrate proficiency in using electronic medical records and telepsychiatry platforms.
• Provide clinical summaries for inpatient admissions with clear purpose and need for hospitalization.
• Complete required legal documentation and understand patient rights and guardianship laws.
• Develop safe discharge plans and coordinate aftercare to promote recovery and prevent readmission.
• Maintain timely documentation of assessments, interventions, and discharge planning.
