Job description

This is a per diem position with a need to cover at least 4 hours a week on Tuesday plus any variable coverage needs. This position coverage for any of our Medication Clinics: Manchester, Londonderry and Raymond. Float.

About the Job

Performs duties in support of the Elliot Anticoagulation Management Program. Collaborates with physicians to provide cost effective, quality services to the patients requiring anticoagulation therapy.

What You’ll Do

  • Provide an anticoagulation management service to EPN/EPS physician network.
  • Obtain PT/INR results from finger stick method or from another source. Complete patient assessment at each visit to include but not be limited to: signs/symptoms of bleeding, signs/symptoms of thrombotic events, significant dietary or medication changes, compliance with the anticoagulation regime, side effects/drug intolerance to anticoagulation therapy, falls, gastrointestinal changes.
  • Accurately adjust anticoagulation dosing within prescribed guidelines using INR reading and clinical assessment as above. Document clearly all dose adjustments and provide documentation to patient at time of visit.
  • Provide educational sessions to outpatient anticoagulation therapy patients and family members to foster adherence, safety and positive clinical outcomes. Utilize written educational materials to provide education.
  • Correctly identify those patients/families requiring additional knowledge, skill or competency to effectively manage the anticoagulation therapy plan. Provide ongoing education and support in an atmosphere that promotes caring and learning.
  • Promptly and accurately identify those situations requiring midlevel or physician supervision, guidance, advice or intervention.
  • Promptly triage anticoagulation patient/family member phone calls. Document all calls/interactions in the anticoagulation navigator in EPIC.
  • Facilitate continuity of care for patients during brief interruptions in anticoagulation therapy (i.e. travel or hospitalization). This may also include providing recommendations for anticoagulation bridging therapy around a procedure when oral anticoagulation must be interrupted.

Educational Needs

Education:Graduate of an accredited school of nursing. Nurses hired after May 2015, must have a bachelor’s degree in nursing or higher nursing degree or commit to achieving the degree within 5 years.

Experience:Cardiology and/or patient medication education preferred.

Certification/Licensure:New Hampshire Registered Nurse or Compact State Registered Nurse License. Obtain BCLS qualification during orientation period and maintain during employment. Certified in Anticoagulation Therapy Management or obtain within 180 days of hire. New hires without certification are required to take the closest available Anticoagulation Therapy Management course to their date of hire.
