The Encore at Boca Raton Rehabilitation and Nursing Center seeks a skilled Speech-Language Pathologist to join our rehabilitation team, delivering evaluation and treatment to short-term and long-term residents. You will develop and monitor individualized goals, document progress, educate patients and families, and collaborate with the interdisciplinary team to ensure compassionate, high-quality care.

A Florida license is required and experience in geriatric care is preferred.

Competenze

• Must have graduated from an accredited Speech-Language Pathology program.
• Current state license and registration to practice in Florida.
• Experience in long-term care or geriatric settings preferred.
• Strong documentation and charting skills.
• Experience with evaluations, progress notes, recertifications, and discharge plans.
• Ability to collaborate with all disciplines to provide patient-centered care.
• Excellent patient service and clinical excellence.
• Punctual, reliable, and professional in scheduling and duties.
• Compliance with confidentiality and HIPAA.
• Familiarity with Case Mix Index and PPS scheduling preferred.

Mansioni

• The Speech Language Pathologist is responsible for the assessment of referred patients requiring rehabilitation.
• Provide direct patient care and determine goals based on patient capacity and tolerance.
• Establish and administer a treatment program under physician direction.
• Set realistic goals and document patient status; educate patients and families.
• Collaborate with all disciplines to ensure best patient-centered care.
• Maintain documentation, progress notes, and discharge planning.
