You will conduct real-time screenings, assist with applications, gather documentation, and coordinate referrals, working with Care Coordinators and Financial Counseling to reduce barriers and improve payer conversion.

Qualifikationen

• Education level equivalent to completion of four (4) years of high school preferred
• College level courses helpful
• Formal Training: Completion of Hometown Health University online classes required
• Work Experience: Prior collection experience working in a hospital/healthcare billing setting preferred

Aufgaben

• Support access, enrollment, and financial navigation by identifying uninsured, self-pay, underinsured, or financially vulnerable patients who may qualify for Medicaid, hospital financial assistance, medication assistance, or other community-based resources.
• Conduct real-time eligibility screenings and assist patients with application and recertification processes.
• Gather required documentation and coordinate referrals to Medicaid, SNAP, prescription assistance, and other social support programs.
• Collaborate with IP/ER Care Coordinators, Patient Access, Financial Counseling, and Centralized Scheduling teams to reduce barriers to follow-up care, prescriptions, and preventive services.
