Job summary

Riverside Community Care in Milford, CT is seeking an Elder Care Transition Specialist to coordinate assessments, help develop treatment plans, and support aging-in-place strategies for clients.

You will collaborate with an integrated team and community providers, ensuring smooth transitions and effective resource connections for individuals choosing to stay in their own communities. A valid driver’s license and a Bachelor’s degree are preferred.

Qualifications

• Bachelor’s degree preferred.
• At least three years working within elder care, either in the community or a facility.
• Degree in nursing, occupational therapy, or social work is preferred.

Elder Care Transition Specialist

ACCS provides wraparound support through an integrated team of both clinical and peer professionals. The goals of the service include skill and resource development, connection with community and natural supports for successful tenure in the community of the individual’s choice.

Responsibilities include:

• Conduct assessments in collaboration with team Nurse, Occupational Therapist, Clinician as needed and assist in establishing treatment plans.
• Consult with the team on necessary elements to successful ‘aging in place’
• Build collaborative relationships with area nursing facilities, rest homes, senior services, etc.
• Provide consultation, training, education and resources to families, treatment team, and ACCS staff.
• Establish and maintain collaborative relationship with DMH Nurse Manager
• Develop a through understating of PASSR regulations and requirements

Schedule: Full Time, 40 hours

Pay Rate: $25.00/hourly

Required Skills
