Job summary

Anderson Elder Law seeks an experienced Elder Care Coordinator to join our interdisciplinary team in Media, Pennsylvania. This role supports older adults and families through care planning, housing decisions, Medicare, public benefits, and transitions across the continuum of care, in collaboration with elder law attorneys.

We value seasoned professionals who communicate clearly, navigate complex aging scenarios, and sustain relationships with clients as circumstances evolve.

Qualifications

• At least three years of substantive professional experience with older adults and families; five or more years preferred.
• Experience in geriatric care management, social work, nursing-home administration, senior living, skilled nursing, rehabilitation, hospice, or related field.
• Strong knowledge of the continuum of care and how families navigate it.
• Excellent assessment, problem-solving, advocacy and communication skills.
• Strong organization, documentation and follow-through.
• Ability to work independently in the community while collaborating with attorneys and other professionals.
• Working knowledge of Medicare and familiarity with Medicaid or other public benefits affecting older adults.
• Local travel is required.

Anderson Elder Law is seeking an experienced aging professional to join our interdisciplinary Elder Law and Life Care Planning team in Media, Pennsylvania.

This is not traditional case management. Our Elder Care Coordinators work alongside elder law attorneys to help older adults and families navigate complicated decisions involving care, housing, Medicare, public benefits, long-term care, family dynamics and transitions across the continuum of care. The relationship does not end when the immediate crisis passes: our ECCs remain involved as circumstances change and the family’s plan evolves.

What You Will Do

• Assess changing care needs, risks, support systems and living options.
• Help families evaluate and implement home care, senior living, rehabilitation, skilled nursing, memory care and hospice options.
• Advocate with healthcare providers, facilities and care communities.
• Participate in care conferences and difficult family decision-making.
• Help clients understand Medicare coverage issues, Part D/prescription concerns and available public-benefit resources.
• Identify Medicaid, Veterans-benefit and related planning issues and coordinate them with our legal team.
• Support family caregivers and surrogate decision-makers.
• Maintain an ongoing relationship with clients as needs and circumstances change.
• Build productive relationships with aging-services and healthcare professionals in the community.

Who We Are Looking For

We are looking for a seasoned professional who can walk into a complicated aging situation, understand what matters, communicate clearly with the family and move the situation forward.

• At least three years of substantive professional experience with older adults and families; five or more years is strongly preferred.
• Experience in geriatric care management, social work, nursing-home administration, senior living, skilled nursing, rehabilitation, hospice, community aging services or a related field.
• Strong knowledge of the continuum of care and how families actually navigate it.
