Senior Community Care of North CarolinaPACE is seeking an Eligibility Specialist in Durham, NC. This role focuses on helping participants secure Medicaid eligibility and navigate public assistance programs, including SNAP, with a patient-centered approach.

The position emphasizes collaboration with Enrollment Specialists, adherence to Medicaid rules, and care coordination for seniors, in a dynamicPACE setting that serves the elderly in the Durham area.

Qualifications

• Bachelor's degree in health and human services or equivalent with 4+ years' related experience.
• 2+ years in case work for public assistance or information/referral in senior services.
• Familiarity with benefits for older adults; appropriate training or related experience.
• At least 1 year experience working with frail or elderly populations.

Responsibilities

• Collaborate with Enrollment Specialist to gather documents and assist Medicaid eligibility applications.
• Ensure compliance with State Medicaid rules for Long Term Care and assist with SNAP applications.
• Track private pay/Medicare-only clients and support transitions to Medicaid eligibility; explain financial responsibilities.
• Coordinate with Health Plan Operations and Executive Director to monitor A/R aging and debt collection.

Education

Bachelor's degree in health and human services or equivalent
