Job summary

Palms Medical Group, a not-for-profit FQHC serving North Florida communities, offers an EHR Optimization Coordinator to bridge clinical operations and quality improvement. You will optimize electronic workflows, support quality measures, improve documentation accuracy, and develop programs that enhance patient outcomes and value-based care.

As part of a collaborative team, you will lead EHR workflow design, training, and audits, with emphasis on NextGen and CQI principles, contributing to

Qualifications

  • RN/LPN/Health Informatics degree or related field.
  • NextGen experience highly preferred.
  • Experience with population health programs.
  • Proficient with Word, Excel and PowerPoint.

Responsibilities

  • Evaluate electronic workflows and population health data.
  • Identify barriers to preventive care completion.
  • Design and collaborate with departments to implement standardized electronic workflows.
  • Support quality improvement initiatives.
  • Assist with revision maintenance of EHR templates.
  • Optimize EHR clinical decision support tools.
  • Improve EHR quality measure documentation workflows.
  • Support EHR risk-adjustment documentation initiatives.
  • Develop provider and staff training programs on optimal population health/quality related EHR workflows.
  • Collaborate with the training department and Performance Improvement Team to provide training as needed.
  • Conduct EHR/EPM workflow and documentation audits.
  • Support adoption of EMR/Population Health best practices.
  • Improve electronic capture of quality measures.
  • Support HEDIS, UDS, and payer initiatives.
  • Help improve quality incentive performance.
  • Other duties as assigned by the Chief Clinical Compliance Officer and/or the CEO.

Position Summary

The EHR Optimization Coordinator (EHROC) serves as the liaison between clinical operations, quality improvement, population health, and the electronic health record. This position is responsible for optimizing electronic workflows, supporting quality measure performance, enhancing documentation accuracy, and developing programs that improve patient outcomes and value-based care performance.

Description of Primary Responsibilities
  1. Evaluate electronic workflows and population health data.
  2. Identify barriers to preventive care completion.
  3. Design and collaborate with applicable departments to implement standardized electronic workflows.
  4. Support quality improvement initiatives.
  5. Assist with revision maintenance of EHR templates.
  6. Optimize EHR clinical decision support tools.
  7. Improve EHR quality measure documentation workflows.
  8. Support EHR risk-adjustment documentation initiatives.
  9. Develop provider and staff training programs on optimal population health/quality related EHR workflows.

  2. NextGen experience highly preferred.

  4. Employee will be expected to navigate and operate Microsoft Office suite products, including Word, Excel and PowerPoint.
  5. Knowledge of CQI principles, practices, methods and tools.
  6. Ability to analyze QI data and identify trends and corrective actions.
  7. Knowledge of clinical quality measures.
Licenses & Certifications
  1. Current BLS (basic life support) Certification

  1. Effectively and tactfully communicates complex and/or technical information to co-workers, patients and/or vendors of various educational, socioeconomical and cultural backgrounds.
