Job summary

MercyOne in Des Moines, IA seeks a Clinical Pharmacist to optimize pharmacotherapy and promote safe patient care within interdisciplinary teams across med-surg, telemetry, oncology, pediatrics, critical care or emergency medicine.

You will perform daily medication therapy reviews, consults, and clinical services, participate in emergencies, mentor students, and ensure policy-compliant practice while pursuing professional certification.

Qualifications

• Pharmacy degree (BS or Pharm.D.) from ACPE-accredited college.
• ASHP-accredited PGY-1 required or 3 years hospital experience.
• PGY-2 residency or 3 years relevant clinical experience preferred.
• BLS certification within 3 months; ACLS or PALS within 3 months based on area.
• Licensed or eligible to practice in Iowa; licensure within 3 months.

Responsibilities

• Assess patient data to identify medication-related problems via profile reviews and rounding.
• Participate in medical emergencies and formal alerts (trauma, stroke, sepsis, STEMI).
• Review orders for appropriateness by age/weight/comorbidity; check for interactions.
• Consult with providers and nurses to optimize pharmacotherapy across care continuum.
• Provide timely drug information and contribute to formulary compliance.
• Serve as a preceptor for pharmacy students and residents.
• Assist nursing with medication administration as assigned, per policy.
• Document therapy plans, monitoring, and follow-up in EMR per guidelines.
• Maintain compliance with state/federal laws and hospital policies.
