Qualifications

• Pharm.D. degree from ACPE approved college required.
• ASHP-accredited PGY-1 Residency or 3 years hospital experience required.
• Pennsylvania Pharmacy license required.
• BLS and ACLS certification prior to orientation.
• Preferred: Pyxis MedStation, EPIC experience.

Responsibilities

• Assess patient data to identify medication problems via reviews and rounds.
• Participate in medical emergencies (trauma, stroke, sepsis, STEMI) as needed.
• Review provider orders to ensure appropriate drug, dose, and form for age/weight/comorbidities.
• Consult with providers/nurses on pharmacotherapy optimization for patient outcomes.
• Provide timely responses to drug information requests and policies.
• Document care activities in the EMR per guidelines.

Location: Mercy Fitzgerald Hospital - Darby, PA

Position Purpose

We are hiring Pharmacists to support operations and our expanded emergency pharmacy program! We are expanding our shift coverage to include a decentralized pharmacist. We are searching for per diem help to cover this role as well as centralized distribution. This position is for pharmacists with experience in hospital pharmacy who are able to work directly with patients for medication histories, counseling, and responding to emergencies within the hospital.

As a Pharmacist, you will:

• Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacy consults and interdisciplinary rounding.
• Participates in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI as required.
• Reviews and interprets providers orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, and comorbidities. Checks for drug incompatibilities and allergies, and takes appropriate action when necessary. Updates medication history for all admitted patients.
• As necessary, consult with providers, nurses and other healthcare professionals involved in patient care regarding pharmacotherapy optimization with intent to achieve optimal patient outcomes across the continuum of care.
• Provides accurate and timely responses to other healthcare professionals and patients regarding drug information questions and facility medication use policies to assure safe and appropriate use of medications.
• Documents patient care activities including therapy plan, monitoring, and follow-up in electronic medical record as per departmental guidelines.

Minimum Qualifications

• Pharmacy degree (Pharm.D.) from an ACPE approved college of pharmacy required.
• Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience is required.
• Registered pharmacist in Pennsylvania required.
• Preferred: Experience with Pyxis MedStation, EPIC
• BLS and ACLS certification before completion of orientation (will be provided if cannot provide upon hire)
