Job Summary:

The medication history technician will obtain medication histories for:

  • Patients to be admitted through the emergency department
  • Newly admitted patients who require a medication history interview
  • Upcoming surgery patients (interview by telephone)

The technician will consult various sources to ensure a complete and accurate medication history. Interviews with patients and/or caregivers are supplemented with review of retail pharmacy records and hospital medication administration records (MARs).

Job Functions:

  • Obtain complete and current medication histories from multiple sources
  • Ensure timely completion of medication histories for physician review
  • Conduct interviews with patients or caregivers to gather medication details, including OTC, herbal, and dietary supplements
  • Communicate with pharmacies, physicians, insurance companies, and third-party software to obtain medication information
  • Document medication histories accurately in the electronic health record for review and reconciliation
  • Coordinate with healthcare professionals to identify high-priority patients for medication history interviews
  • Resolve medication history discrepancies and consult pharmacists or physicians as needed
  • Conduct quality audits of medication histories
  • Assist with emergency department medication distribution and perform monthly audits
  • Prioritize workload to meet patient care needs efficiently

Experience Requirements: 3 years pharmacy technician experience in a hospital setting preferred.

Educational Requirements: High school diploma or equivalent.

Special Skill, License, and Knowledge Requirements:

  • National PTCB (CPhT) licensure required upon hire
  • State of Louisiana Pharmacy Technician Licensure required upon hire
  • Pharmacy Technician Career Ladder assessment within 60 days of hire
  • CPR/BLS certification within 60 days of hire
  • Two years technician experience in retail or hospital setting required

HIPAA Requirements: Maintain knowledge of and adherence to HIPAA regulations related to patient records, information, and privacy.

Safety Requirements: Follow safety practices including incident reporting, waste handling, PPE use, and environment of care procedures.
