Job summary

Insight Global is seeking a Medical Coder with at least three years of Emergency Department coding experience to review hospital records and assign ICD-10-CM/PCS, CPT, and HCPCS codes for inpatient and outpatient settings. The role supports accurate reimbursement and regulatory compliance.

You will apply coding guidelines, submit provider queries, and perform chart reviews to ensure documentation quality. Strong ED coding knowledge and communication skills are essential.

Qualifications

• 3+ years of recent Emergency Department coding experience.
• Strong knowledge of ICD-10-CM, CPT, and HCPCS coding guidelines.
• Experience coding inpatient observations.
• Experience submitting provider queries and researching documentation discrepancies.

We are seeking Medical with at least three years of experience. The Inpatient Coder will be responsible for reviewing hospital patient medical records and assigning accurate diagnostic or procedural codes (ICD-10-CM/PCS, DRGs) to ensure proper reimbursement, accurate data for reporting, and compliance with healthcare regulations.

Key responsibilities:

• Apply appropriate coding classification standards and guidelines to medical record documentation for accurate coding.
• Submit necessary provider queries to resolve documentation discrepancies.
• Perform quality assessment of records, including verification of medical record documentation.
• Responsible for researching errors or missing documentation from medical records to provide accurate coding processes.
• Abstracts and assigns the appropriate ICD-10-CM/PCS codes for all diagnoses and procedures performed in the outpatient and inpatient settings as applicable.

Skills and Requirements

• 3+ years of recent Emergency Department (ED) coding experience
• Strong ED Facility and ED Professional Fee (ProFee) coding
• Experience coding inpatient observations.
• Strong knowledge of ICD-10-CM, CPT, and HCPCS coding guidelines
• Experience working coding edits, denials, and documentation review preferred
• Strong communication skills and professionalism
