Job summary

Mass General Brigham is seeking registered nurses for the Case Management department in a busy hospital setting at 55 Fruit Street, Boston. The Case Manager collaborates to enhance patient care, coordinates planning, and advocates along the continuum of care to ensure quality outcomes.

You will participate in orientation programs, including the Novice to Expert pathway, and join a supportive leadership team committed to diversity, inclusion, and compassionate care.

Qualifications

• RN Case Manager experience required.
• Must be a licensed RN in the state of Massachusetts.
• Case Manager certificate preferred.
• ADN required, but BSN strongly preferred.
• Acute hospital experience preferred.

Responsibilities

• Collaborate to enhance the delivery of patient care services along the continuum of care.
• Meet patient's needs efficiently and expeditiously by continuously improving the patient's experience.
• Evaluate, predict, and facilitate the trajectory of patient care.
• Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation.
