Complex Care ECM (Enhanced Care Management) Data Coordinator

Report To: Associate Director of Complex Care

  • Detail-oriented and highly organized professional with experience managing data, maintaining accurate records, and ensuring quality assurance across complex workflows in fast-paced environments.
  • Proficient in Microsoft Excel, Google Sheets, databases, and data management systems, with strong analytical skills and the ability to identify, reconcile, and resolve data discrepancies efficiently.
  • Collaborative communicator with a commitment to confidentiality and compliance, skilled at working cross-functionally to support healthcare operations, reporting, member services, and process improvement initiatives.
Role Summary

The Enhanced Care Management (ECM) Data Coordinator supports day-to-day data operations for SFCHC’s Complex Care ECM programming by managing and monitoring member data throughout the full-service lifecycle. The Data Coordinator is responsible for end-to-end data management including referrals, member information files, return transmission files, authorization and billing support, data systems maintenance, reporting, and compliance tracking. The ideal candidate is detail-oriented, organized, comfortable working with spreadsheets and databases, and interested in healthcare operations, managed care, and population health programs.

  • Receive, review, track, and process incoming ECM referrals within SFCHC, health plans, and community partners.
  • Validate referral completeness, eligibility, and required documentation.
  • Enter and maintain accurate referral and member records in internal databases and care management systems.
  • Monitor referral status workflows and ensure timely assignments and follow-up.
  • Communicate with internal teams (such as ECM Program Managers, Lead Care Managers, and Eligibility Specialists) regarding missing or incomplete referral information.
  • Import, update, reconcile, and maintain member eligibility and demographic files internally within SFCHC’s OCHIN Epic.
  • Review member rosters for accuracy, duplicates, eligibility discrepancies, and enrollment changes.
  • Maintain accurate records of member participation, service dates, and program status.
  • Manage monthly member information reconciliation with Managed Care Plans (MCPs), as well as ad hoc member file processing and quality assurance activities.
  • Validate file formatting, completeness, and transmission timelines.
  • Research and resolve rejected records, file errors, and data discrepancies.
  • Maintain logs and documentation of all outbound and inbound file transmissions.
  • Manage SFCHC ECM Billing Workque, as well as assist with tracking service authorizations, billing documentation, and claims-related data.
  • Verify service records against authorization requirements and payer guidelines.
  • Support reconciliation of billed services and internal service logs.
  • Collaborate with finance and operations teams to resolve billing discrepancies.
  • Maintain data integrity across multiple systems, spreadsheets, and reporting tools.
  • Assist with user audits, system updates, and data cleanup activities.
  • Generate routine operational reports and dashboards for leadership and program teams.
  • Identify opportunities for workflow improvement and process standardization.
  • Support implementation and maintenance of ECM-related databases and tracking systems.
  • Conduct regular quality assurance reviews to ensure accuracy and completeness of records.
  • Support internal audits, compliance reviews, and reporting requests.
  • Follow established policies and procedures for confidential health information.
Minimum Qualifications
  • Bachelor’s degree in Healthcare Administration, Public Health, Business Administration, Information Systems, or related field preferred (or equivalent experience).
  • Minimum 2 years of administrative, healthcare operations, data entry, or coordination experience.
  • Experience working with healthcare data, managed care, Medi-Cal, or CalAIM Enhanced Care Management programs preferred but not required.
  • Experience using EHRs (particularly OCHIN Epic), care management platforms, CRM systems, or billing systems.
  • Strong attention to detail and organizational skills.
  • Proficiency in Microsoft Excel, Google Sheets, and data entry systems.
  • Ability to manage multiple tasks and meet deadlines in a fast-paced environment.
  • Strong written and verbal communication skills.
  • Ability to maintain confidentiality and handle sensitive information appropriately.
  • Analytical problem-solving and data reconciliation skills.
  • Ability to work independently and collaboratively across departments.
  • Knowledge of HIPAA and healthcare compliance standards.
  • Bilingual or multilingual skills ideal but not required.
  • Regular on-site work within clinic settings.
  • Combination of field-based patient engagement and administrative/reporting responsibilities.
  • Requires up-to-date vaccinations, especially COVID-19 and flu shot vaccinations; as well as proof of TB clearance (issued within the past 12 months).
