Job summary

BlueCross BlueShield of Tennessee, Inc. seeks a dedicated Support Coordinator to assist individuals with I/DD through ECF Choices in Greene and Washington Counties, TN.

You will conduct intake, enrollments and ongoing assessments, develop plans, and coordinate health, behavioral health and community supports. The role requires a valid driver’s license, 2+ years in healthcare operations, and relevant certifications or education (RN/CDDS/CDDN or QDDP; MSW/LCSW preferred).

Qualifications

• RN with current CDDS/CDDN for RNs or a DDC for LPNs, or BS in social work, nursing, education, health care or related field.
• Licensed MSW or LCSW preferred; QDDP or QIDP eligibility meets federal requirements.
• 2 years of experience in healthcare or operations environment required.
• 5+ years as independent Support Coordinator or case manager for HCBS Waiver and completion of Personal Outcome Measures Introduction and Assessment Workshop trainings.
• Valid driver’s license; willingness to travel to members' homes.

Responsibilities

• Partner with members and families to identify needed supports and direct services to meet goals for health, employment and independent living.
• Collaborate with clinical and social support teams to meet physical, behavioral health and long-term service needs.
• Conduct thorough face-to-face visits and assess member needs across physical, behavioral, functional and financial domains.
• Authorize and verify home and community-based services are provided and meet member needs.
• Maintain driver’s license validity and comply with vehicle safety policies as required.

ECF Choices is seeking a dedicated Support Coordinator to join our team in Greene & Washington County. ECF CHOICES is a program in Tennessee designed to support individuals with intellectual and developmental disabilities (I/DD). The program aims to help these individuals live as independently as possible and become employed within their communities. In this role, you will holistically support our members’ physical, behavioral, and social needs, helping them achieve their goals and live their dreams. ECF CHOICES plays a crucial role in the Tennessee community, and you will have the opportunity to positively impact numerous lives.**Key Responsibilities:*** Empower members by supporting their health and well-being.* Assist members in finding and maintaining competitive, integrated employment.* Navigate challenging relationships to ensure members receive necessary services.* Perform intake, enrollment, and ongoing assessments.* Develop and maintain Person-Centered Plans.* Monitor and maintain overall service provision.* This role supports the Greene, Washington County area and will require traveling to **members’ homes** (mileage is reimbursed)* Available for an 8:00am – 5:00pm (no on call) schedule.* Preference for candidates to live in: Greene or Washington County**Job Responsibilities*** Partnering with members and families to identify needed supports and direct services to meet personal goals for good health, employment and independent or community living.* Collaborates with a team of clinical and social support colleagues to meet the physical, behavioral health and long term service needs of each member.* Conduct thorough and objective face-to-face visits with and assess each members situation to determine current status and needs, including physical, behavioral, functional, psycho-social, financial, and employment and independent living expectations.* Utilizing criteria for authorizing appropriate home and community based services and confirm those services are being provided and that members needs are being met.* Valid driver's license.* Various immunizations &/or associated medical tests may be required for this position.* This job requires digital literacy assessment.**Job Qualifications***Education** RN, with a preference that such individuals also have current Certification from the Developmental Disabilities Nurses Association as a Certified Developmental Disabilities Nurse (CDDN) for RNs or a Developmental Disabilities Nurse (DDC) for LPNs, as applicable;* Or B.S. in social work, nursing, education, human service or health care profession or other related field; Licensed Master Social Worker or Licensed Clinical Social Worker Preferred;* Or meet the federal requirements for a Qualified Developmental (or Intellectual) Disabilities Professional (QDDP or QIDP);* Or in lieu of a degree, have 5+ years’ experience as an independent Support Coordinator or case manager for service recipients in a 1915(c) HCBS Waiver and have completed Personal Outcome Measures Introduction and Assessment Workshop trainings as established by the Council on Quality and Leadership.*Experience** 2 years - Experience in the healthcare industry or in an operations type environment required.*Skills\\Certifications** Ability to work independently with minimal supervision or function in a team environment sharing responsibility, roles and accountability.* Proficient in Microsoft Office (Outlook, Word, Excel and PowerPoint)* Capacity to solve problems and manage multiple assignments with critical deadlines* Must be a team player, be organized and have the ability to handle multiple projects* Excellent oral and written communication skills* Strong interpersonal and organizational skills* Be organized, reliable, and able to manage multiple tasks with exceptional work ethic.Employees who are required to operate either a BCBST-owned vehicle or a personal or rental vehicle for company business on a routine basis\\* will be automatically enrolled into the BCBST Driver Safety Program.
