Job summary

Transforming Age is seeking a dedicated Certified Nursing Assistant to support residents at our Lincoln, NE facility. You will follow individual ADL directives, document conditions, assist with daily activities and meals, and help with admissions and discharges.

The role requires a valid CNA license, CPR/First Aid certification, and experience with EHR systems. Excellent English communication and the ability to work collaboratively are essential.

Qualifications

• Current CNA license in good standing.
• Medication Aide / CMA certification as required by the state.
• Current CPR/First Aid certification.
• Proficient reading, writing, and speaking in English.
• Experience with electronic point of record/electronic health record software.
• Pass required background checks, physical, TB test, and other pre-employment screenings per state/local guidelines.

Responsibilities

• Review and follow residents’ individual ADL directives.
• Make accurate daily notes on residents’ conditions and chart them on flow sheets.
• Report changes to the supervisor.
• Assist residents with toileting, oral hygiene, bathing, grooming, nail clipping, dressing, and serving meals.
• Lead and attend activities as needed.
• Assist with admissions, discharges, and check schedules to support residents.
• Record vital signs and conditions on a scheduled basis or as needed.
• Provide verbal and physical assists to residents during transfers and ambulation.
• Respond promptly to call lights and report safety hazards.
• Maintain neat and clean work areas and resident rooms.
• Provide residents with opportunities to participate in social, physical, creative, and spiritual activities.
• Administer daily medications in accordance with the prescribing clinicians’ guidance.
