Qualifications

• Reliable transportation and a valid driver’s license.

Responsibilities

• Coordinate care services for clients in the East Cobb Region, including matching clients with suitable caregivers and scheduling appointments.
• Conduct client assessments to understand needs, preferences, and goals, and develop personalized care plans.
• Ensure care services are delivered safely, efficiently, and effectively.
• Communicate with clients, families, and caregivers to address concerns.
• Maintain accurate records of client interactions, care plans, and service delivery.
• Collaborate with management to improve client satisfaction and care outcomes.
• Participate in quality improvement initiatives to meet high standards.
