Job summary

Sevita's Mentor South Bay in Massachusetts is seeking an Early Intervention Physical Therapist to coordinate treatment and implement services as part of a multidisciplinary Early Childhood Services team.

Responsibilities include outreach to children and families, developing individualized plans with the team, maintaining complete IFSP records, and ensuring compliant billing and documentation while safeguarding confidentiality.

Qualifications

• Current licensure as a Physical Therapist by the Massachusetts Board of Allied Health Professions.

Responsibilities

• Provide outreach services to children and families
• Consult with team members in the development of individualized plans and multidisciplinary services for assigned clients
• Maintain client records with IFSPs and goals, including assessments and progress notes
• Documentation of client progress and required collateral/family contacts
• Complete billing documentation according to schedule
• Maintain confidentiality and comply with clinical and billing standards
• Conduct visits with clients in natural environments according to needs
