VOLUNTEERS OF AMERICA SOUTH CENTRAL LOUISIANA is seeking a Care Coordinator for the Child First program in Lafayette, LA. You will partner with a Mental Health and Developmental Clinician to connect families with community services and supports, strengthen caregiver skills, and promote healthy child development.

Qualifications

• Bachelor’s degree in child development, psychology, nursing, human services or related field preferred.
• Minimum of three years working with culturally diverse families and young children.
• Good verbal and written communication skills; comfortable with computers.

Responsibilities

• Provide community resource expertise to Child First team and families.
• Engage with families and Clinician to complete assessments and identify needs.
• Promote family stabilization by integrating service needs into care plans.
• Enhance caregiver executive functioning skills as needed.
• Maintain reflective stance and participate in supervision.
• Track assessments and maintain documentation for accountability and reimbursement.
• Coordinate home visiting activities and participate in training and collaborative learning.
