St. James Rehabilitation and Healthcare Center is seeking a skilled Speech Language Pathologist (SLP) to provide and direct Speech Therapy services. Responsibilities include assessment, treatment implementation, program development, and education under physician orders.

Collaboration with the care team and patient-family communication are essential. The role involves planning and coordinating care, monitoring progress, and supporting discharge and community reintegration.

Qualifications

• Masters Degree in Speech-Language Pathology.
• Current state licensure or license eligible.
• Demonstrates good communication skills.
• Maintains education and development appropriate for position.
• Works well with others in a spirit of teamwork.
• Performs position-specific functions at an acceptable level.
• Supports health system mission, vision and values.

Responsabilités

• Provides Speech Therapy services to patients and initiates referrals.
• Conducts screenings of patients to determine need for intervention.
• Evaluates patients for treatment planning and implements plans.
• Communicates evaluation findings to patients, family, and team members.
• Develops treatment plans with goals and methods.
• Coordinates treatment plans with patient and family.
• Monitors progress and adjusts treatment as needed.
• Assists with discharge planning and home/community programming.
