Qualifications

• Licensed Physical Therapist (PT) with current license (IDFPR) and Basic Life Support (BLS) within 30 days.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair or floor.
• Uses safe body mechanics to protect patients and staff while delivering care with dignity and respect.
• Designs an appropriate plan of care integrating goals, treatment, outcomes and discharge planning.
• Adjusts care based on patient response and evidence.
• Educates patients, family, caregivers, and other health care providers using effective teaching methods.
• Evaluates findings to determine PT diagnoses and outcomes of care.
• Performs examinations using reliable methods relevant to chief complaint, screening and history.
• Screens patients to determine need for therapy services according to standards.
• Monitors the care delivered by support personnel and provides feedback.
• Communicates with medical team.
• Performs documentation per department standards.
• Meets customer service and productivity standards.
• Serves as clinical instructor when assigned by team lead/manager/supervisor.
• Participates in team and department activities and meetings.
• Performs reporting and other duties as assigned.
