Job summary

Kaiser Permanente in California seeks a licensed Physical Therapist to manage the therapy care for patients. You will work under the Director/Clinical Supervisor to evaluate, plan, implement, and discharge care within Spine Care and general PT services.

Responsibilities include interpreting examinations, establishing care plans, administering modalities and exercises, documenting per policy, and coordinating with a multidisciplinary team to restore function and independence for patients.

Qualifications

• Completion of an accredited PT program with a Bachelor's, Master's, or Doctorate in Physical Therapy.
• California Physical Therapist licensure required at hire or within four months.
• Basic Life Support (BLS) certification required at hire.
• National Provider Identifier (NPI) required at hire.

Responsibilities

• Evaluates patients and interprets examination findings to determine the nature and extent of dysfunction and to restore movement and function.
• Establishes plan of care to achieve established goals relative to age and medical history.
• Administers modalities and other physical agents.
• Administers or prescribes exercise programs and patient care development.
• Coordinates discharge planning with other health care personnel and participates in rounds as appropriate.
