Job summary

Flagstar Nursing is seeking Physical Therapists for Skilled Nursing Facilities across NYC and Long Island. PTs evaluate, treat, and educate patients to improve function in SNF settings, collaborating with a multidisciplinary team.

We offer Direct Employment or Independent Contractor options, with weekly pay and corporation setup assistance. Pay rates are $46-$48/hr (W-2) or $57-$60/hr (1099).

Qualifications

• Current NYS PT License and Registration.
• CPR Certification.
• EMR competence and familiarity with SNF workflows.
• Meets all State and Federal requirements for US employment in a nursing facility.
• Malpractice/liability insurance (if applicable).

Responsibilities

• Provide efficient evaluations and create individualized treatment plans.
• Supervise PTAs daily.
• Treat patients in SNF settings for diverse conditions.
• Monitor and document patient progress using EMR software.
