Qualifications

• Licensed Physical Therapist in Illinois (or eligible)
• Experience in skilled nursing rehab preferred
• Ability to assess and develop personalized treatment plans

Responsibilities

• Assess residents and craft personalized treatment plans focused on pain relief and functional gains
• Lead therapy sessions to improve strength, balance, and mobility
• Collaborate with staff, families, and the therapy team to drive progress and positive outcomes
