Qualifications

• Licensed Physical Therapist with active state license.
• Experience in evaluating movement dysfunction and developing treatment plans.
• Ability to supervise PT assistants and rehabilitation staff.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms, review medical history and referrals.
• Diagnose movement dysfunction by observing patients and their movement.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records and track goals.
• Utilize standardized assessments and evidence-based practice to guide interventions.
• Assess progress and adjust treatments in the clinical record.
• Evaluate outcomes and modify services based on patient progress and referrals.
• Provide consultation to patients, families, and other providers.
• Communicate with supervisor and health team about patient progress and plans.
• Provide comprehensive discharge summaries per requirements.
• Document all required information in the electronic system and maintain records.
• Participate in trainings, in-services, and conferences.
• Able to supervise PT assistants and staff.
• Adhere to company policies and code of conduct.
• Active State Licensure.
