Job summary

UF Health Jacksonville seeks a licensed Physical Therapist to evaluate patients, develop and implement individualized treatment plans based on diagnosis, history and age. You’ll monitor progress, adjust plans, document progress notes, reevaluations and discharges, and ensure patient safety and privacy.

You will also educate patients and families, maintain departmental records, and provide community education and support within the department as needed.

Qualifications

• Knowledge of Physical Therapy theory, procedures, practices and equipment.
• Good communication skills with patients and team members.
• Thorough understanding of safety precautions and necessary measures.
• Familiarity with therapy equipment and patient monitoring.
• Ability to assess patient responses to treatment.
• Ability to coordinate a diverse treatment program for multiple diagnoses.
• Computer skills in Word and Excel.

Responsibilities

• Evaluate patients and develop treatment plans based on diagnosis, history, age and examination.
• Observe progress, modify treatment plans, and educate patients on treatment purpose and expected results.
• Document evaluations, progress notes, reevaluations and discharges in medical records.
• Maintain departmental records and comply with regulatory agencies.
• Provide education to patients and the community as needed.
