• Collaborates with physicians, caregivers, and staff on discharge planning.
• Directs treatment responsibilities delegated to other personnel.
• Educates patients on evaluation, treatment, and self-care.
• Instructs home treatment programs to promote independence.
• Responds promptly to messages and phone inquiries.
• Participates in department meetings and performs other duties to ensure PT program success.
