Job summary

Paramount Care Centers is seeking a Physical Therapist (PT) in Hudson Heights, NJ. The position offers hourly and per diem pay with several benefits.

The PT will organize and conduct physical therapy programs to facilitate rehabilitation and promote independence, working to improve mobility and functional outcomes for residents. The role includes evaluating residents, designing treatment plans, and coordinating with the rehab team to provide safe, high-quality care.

Qualifications

• Graduate of a physical therapy curriculum accredited jointly by the AMA Council on Medical Education and the APTA and certified by the APTA.
• Licensed and/or eligible for licensure as required by state.
• Strong documentation and communication skills.

Responsibilities

• Evaluates residents, sets treatment goals, and implements PT treatment plans with proper documentation.
• Designs and fits adaptive devices, and trains residents in their use.
• Selects appropriate physical agents and modalities per resident needs.
• Collaborates with rehabilitation team to choose best regimen.
• Instructs residents in mobility skills and assesses progress.
• Adapts program to individual needs and safety policies.
• Promotes a safe environment and adheres to safety standards.
• Attends care conferences and related meetings with staff.
• Supervises supportive personnel per state standards.
• Conducts training programs in PT techniques for staff.
• Orders supplies and maintains equipment as needed.
• Cleans and maintains equipment and reports repairs when necessary.
• Ensures residents and families receive high-quality service.
