Job summary

Bethesda provides and directs Physical Therapy services for residents across Bethesda's care settings, including SNF, Home Health, and Outpatient programs. You will evaluate, treat, and execute therapy plans under physician orders while documenting in accordance with policy and state regulations.

Collaborating with the rehabilitation team, you will coordinate care plans, educate residents and families, and participate in continuing education.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Current accredited MN Physical Therapist License.
• 1–3 years of prior experience in rehab settings (acute care, inpatient rehab, outpatient, SNF, long-term care, home health).
• Knowledge of State and Federal regulations.

Responsibilities

• Provide and direct therapy services across Bethesda's care settings.
• Evaluate and treat residents/clients and develop evidence-based therapy plans.
• Document progress and communicate status to the care team and families.
• Coordinate treatment plans with residents, families, and team members.
• Participate in ongoing education and maintain professional development.
