Qualifications

Responsibilities

• Completes chart review to determine PT appropriateness.
• Chooses exam and integrates findings into a goal-centered plan of care.
• Uses evidence-based practice to guide therapy intensity.
• Assesses response to intervention and adjusts plan of care.
• Educates patient and family; communicates with interdisciplinary team.
• Documents care clearly to support clinical decisions; ensures safety of patient handling.
• Supports discharge planning and follow-up needs based on patient context.
