Slate Care Solution Physical Therapy in Avenel, NJ is seeking a dedicated Physical Therapist for a full-time onsite role. You will evaluate patients, design individualized treatment plans, and work within a collaborative rehabilitation team to improve mobility and quality of life.

Qualifications

• Bachelor’s Degree in Physical Therapy or Doctorate of Physical Therapy (DPT) from an accredited program.
• Current State License in Physical Therapy, ensuring compliance with all local regulations.
• Exceptional written and verbal communication skills, essential for effective patient interactions and documentation.
• Proficient in utilizing electronic healthcare systems for managing patient records and treatment tracking.
• A strong commitment to patient-centered care and a willingness to adapt to changing patient needs.

Responsibilities

• Conduct thorough evaluations of patients to understand their medical history, physical limitations, and setting appropriate treatment goals.
• Develop and implement individualized treatment plans that utilize exercise, hands-on therapy, and specialized equipment to promote healing and enhance mobility.
• Monitor patient progress, document treatment sessions and outcomes, and modify treatment plans as necessary to ensure maximum effectiveness.
• Educate patients, their families, and caregivers about rehabilitation protocols, home-based exercises, and the correct use of any assistive devices.
• Maintain an organized work environment and adhere to all safety guidelines when using therapeutic modalities and equipment.
• Participate in interdisciplinary meetings, contributing insights from a physical therapy perspective to optimize patient care.
• Engage in continuous professional development by attending workshops, seminars, and by staying current with best practices in physical therapy.
