St. Luke’s Health – Memorial Lufkin, part of CommonSpirit Health, is seeking a Physical Therapist to help patients rebuild and maintain function, reduce pain, and improve quality of life. You will perform initial evaluations, develop individualized care plans, and deliver direct treatments.

Collaborate with the interdisciplinary team and families to meet patient goals, ensuring productive care and excellent customer service. A PT degree and TX licensure eligibility with BLS/CPR are required.

Formación

• Degree in Physical Therapy and graduates eligible for licensure.
• Basic Life Support - CPR (CPR/BLS) required.

Responsabilidades

• Develop and update appropriate plan of care in collaboration with the patient/family and related to the persons age and lifestyle with measurable long and short term goals and a defined D/C plan.
• Perform age appropriate competency skills in area of practice and with respect to individual patient problems; follow intervention protocols and alter plan of care appropriately.
• Discharge patient when goals have been met or medical necessity is no longer warrants intervention; establish provisions and/or make recommendations.
• Consistently meet patient care productivity standards; work with supervisors and peers; participate in patient coverage across divisions.
• Promote professional customer service and collaborate with interdisciplinary team to meet patients' needs in a timely manner.
• Complete all necessary documentation including charting, charge entry, peer reviews, and daily schedule.
