Job summary

Qualifications

Doctorate in Physical Therapy required.
NYS PT license and registration required.
BLS certification required.
Strong communication with patients, insurers and staff.
Communicate with MDs about patient care on an ongoing basis.
Knowledge of EMR, Word/Excel, PT billing codes.
Familiar with manual PT treatments and PT equipment.

Responsibilities

Perform and document an initial exam, evaluating data to identify problems and diagnose prior to intervention.
Record prognosis, treatment, response and progress in patient chart or EMR.
Plan, prepare and carry out individually designed physical therapy programs.
Evaluate effects of treatment and adjust for maximum benefit.
Direct, supervise, assess and communicate with support staff.
Provide education on physical therapy, injury prevention and ergonomics.
Discharge patients when goals are attained and arrange follow-up care.
