Responsibilities

• Perform and document an initial exam, evaluating data to identify problems and diagnose prior to intervention.
• Record prognosis, treatment, response and progress in patient chart or EMR.
• Plan, prepare and carry out individually designed physical therapy programs.
• Evaluate effects of treatment and adjust for maximum benefit.
• Direct, supervise, assess and communicate with support staff.
• Provide education on physical therapy, injury prevention and ergonomics.
• Discharge patients when goals are attained and arrange follow-up care.
