Responsibilities

• Performs patient assessments resulting in an effective treatment plan.
• Reassesses patients at appropriate intervals and modifies the plan of care.
• Involves patient and family in assessment and care planning.
• Provides care per physician orders and ensures quality outcomes.
• Communicates with physicians, nurses, case managers and the health care team.
• Documents outcomes at initial and discharge evaluations.
• Demonstrates knowledge of physical therapy modalities and uses equipment safely.
• Formulates and teaches a plan based on learning needs, involving family when appropriate.
• Maintains documentation standards and timely record completion.
• Participates in continuing education and professional growth.
