Job summary

Sweetwater Care is seeking an Occupational Therapist to evaluate patients, design individualized treatment plans, and guide recovery through evidence-based therapies. You will educate patients and families, supervise assistants, and document progress in the electronic health record with accuracy.

The role requires an active state OT license, strong communication, and collaboration with the rehabilitation team to optimize outcomes and discharge planning.

Qualifications

• Active State Licensure as an Occupational Therapist.
• Experience with rehabilitation and patient education is preferred.
• Ability to document treatment minutes and regulatory information accurately.

Responsibilities

• Consult with patients to assess condition and plan care.
• Diagnose movement dysfunction through observation and history.
• Develop individualized treatment plans and modify as needed.
• Supervise and support PT assistants and rehabilitation staff.
• Provide counseling to patients and families on treatment options.
• Maintain accurate patient records and discharge summaries.
• Communicate progress with the care team and supervisors.
• Ensure compliance with regulatory and clinical practice standards.
