Job summary

Oakland Heights Nursing and Rehabilitation and Lake Park Healthcare Center in Oakland, CA, are seeking a licensed Speech Language Pathologist for a split Part-Time role across two established Skilled Nursing communities. You will evaluate, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders with clinical autonomy.

The position offers flexible part-time coverage, dual-site experience, and meaningful resident outcomes.

Qualifications

• California licensure as a Speech-Language Pathologist is required.
• Master's or Doctoral degree in Speech-Language Pathology is required.
• CCC-SLP credential required if mandated by facility or payer guidelines.
• CPR/BLS certification or willingness to obtain per policy.

Responsibilities

• Conduct comprehensive speech-language and swallowing evaluations per physician orders and CA standards.
• Assess and diagnose speech, language, cognitive-communication and dysphagia disorders.
• Develop and modify individualized plans of care with measurable, function-based goals.
• Provide skilled therapeutic interventions addressing communication, cognition, swallowing safety, and compensatory strategies.
• Monitor patient progress and adjust treatment plans based on outcomes.
• Lead discharge planning and support safe transitions of care.

Education

Master's or Doctoral degree in Speech-Language Pathology

State license: California SLP

CCC-SLP (where mandated)

CPR/First Aid certification (CPR/BLS)
