Job summary

HealthPartners in Burnsville, MN seeks a Registered Nurse to provide direct ambulatory patient care and coordinate nursing activities within the care team.

The RN assesses, plans, implements and evaluates patient care, guiding and supervising assistive personnel while ensuring accurate documentation and continuity of care. Compliance with Nurse Practice Act and internal policies is required.

Qualifications

• Associates/Diploma nursing degree required.
• Fulfills Minnesota RN licensure and BLS provider status.
• Knowledge of age-related growth and development for safe patient care.

Responsibilities

• Develop and initiate plan of patient care using an outcomes-based approach.
• Coordinate, oversee, deliver, and delegate nursing care based on patient assessment/standards of care.
• Direct, evaluate, and supervise the work of assistive personnel in providing delegated nursing services, and development principles.
• Document, reassess and alter plan of care based on patient's response to nursing interventions on an ongoing basis.
• Communicate pertinent patient data to other members of the health care team in a timely fashion.
• Collaborate/coordinate with other health care team members, patient, and family members to determine necessary resources and ensure continuity of care.
• Assess learning needs of patients and provide comprehensive, individualized health care teaching.
• Seek opportunities to incorporate research into daily practice.
• Plans and implements staffing based on patient acuity and census.
