Job summary

Futures Unlimited, Inc. in Pontiac, IL is seeking a Direct Support Person (DSP)/Residential Trainer to assist adults with intellectual/developmental disabilities. You will document progress, support daily living activities, and promote health, safety, self-advocacy, and community inclusion.

This full-time overnight role includes 3 nights one week and 4 nights the next, with every other weekend. You will transport individuals, assist with personal care, follow individual service plans, complete

Qualifications

• High school diploma or equivalent.
• Valid Illinois Driver’s License.
• Maintain minimum insurance requirement.
• Pass physical function test.
• Must have or be able to obtain DSP classification and medication training.
• Must have or be able to obtain CPR, Heimlich Maneuver, and First Aid certifications.
• Solid communication and interpersonal skills.

Responsibilities

• Assist in transportation of individuals. Safely transport and monitor the wellbeing of passengers.
• Assist in the personal care and hygiene of persons served.
• Engage and support persons served in daily activities and experiences.
• Initiate and follow individual outcomes as identified in person-centered/individual service plans and as directed by QIDP.
• Complete documentation of goal outcomes.
• Assist in the planning and development of program curriculum, experiences, and activities.

Job Description

Direct Support Person (DSP) assists and supports adults with intellectual/developmental disabilities. This position is responsible for documentation of individual progress, goal progress, behavioral/physical health, and service hours. The DSP plans, initiates, and supports persons served in a variety of social, educational, wellness, and vocational training experiences. This role promotes health and safety, self-advocacy, and community inclusion.

Full-time overnight position.

3 nights one week and 4 nights the other week.

Every other weekend.

Essential Functions

• Direct Support Person/Residential Trainer
  • Assist in the transportation of individuals. Safely transport and monitor the wellbeing of passengers.
  • Assist in the personal care and hygiene of persons served.
  • Engage and support persons served in daily activities and experiences.
  • Initiate and follow individual outcomes as identified in person-centered/individual service plans and as directed by QIDP.
  • Complete documentation of goal outcomes.
  • Assist in the planning and development of program curriculum, experiences, and activities that address the needs and desired outcomes of persons served.
  • Follow behavior support plans as identified in individual service plans.
  • Be accountable for the location and activity of assigned individuals.
  • Assist and support individuals at meal times, promoting independence, safety, and wellness.
  • Document required transportation information and adhere to transportation policies.
  • Complete documentation daily on individuals’ progress and participation.
  • Complete documentation of behavioral and physical health.
  • Be alert to safety/medical needs of individuals, provide first aid, and contact supervisor as needed.
  • Communicate behavioral incidents to supervisor and/or QIDP.
  • Communicate physical health concerns to Health Services Manager.
  • Complete appropriate incident/injury, critical incident, and seizure reports within the timeframe directed by supervisor.
  • Meet regularly and communicate with supervisory staff, sharing information and ideas to address individual needs and contribute to the functioning of the CILA program.
  • Assure that necessary chores related to the routine care and maintenance of the home are completed.

Other Responsibilities

• Attend trainings to meet annual requirements: First Aid, CPR, CPI, Safety, and OIG.
• Attend agency and house meetings regularly.
• Accurately complete and approve timecards by the deadline.
• Respect persons served and promote a positive image of persons with disabilities.
• Perform other related duties as assigned.

This position is a full-time, hourly, non-exempt position. Hours will vary based on the individual needs of the residence. Evening and weekend hours are to be expected. This individual reports directly to the Residential Home Coordinator.
