Job description

REPORTS TO:
House Manager

Qualifications:
  • High School Diploma or equivalent.
  • Acquire and maintain certification in First Aid, CPR, and CPI (Non-Violent Crisis Intervention). Subject to annual TB test. Valid Driver's license from the state of residence and ability to operate a motor vehicle for 15 passengers or less.
Job Summary:

Provide direct care and training to consumers in community residence according to local, state, and federal guidelines. Implement training goals and collect data. Counsel consumers as necessary.

Physical Abilities Include:

Climbing, balancing, stooping, kneeling, crouching, crawling, reaching, standing, walking, pushing, pulling, lifting (with assistance), fingering, writing, reading, driving (including nights), counting, working double shifts as necessary, running, and smelling.

Physical Requirements Include:

Very heavy lifting with assistance, visual acuity to read small print and record data. Must work in indoor and outdoor environments, possibly exposed to temperature extremes, noise, hazards including physical assault, property destruction, and infections. May need to wear protective clothing to prevent infection exposure.

Job Duties:
  1. Maintain accountability for all consumers within the assigned setting and monitor their actions during that time.
  2. Meet consumers' needs through goal implementation, behavior management, personal hygiene, social adjustment, leisure, and activities.
  3. Ensure consumers' well-being, report maladaptive behaviors, and implement behavior management plans.
  4. Complete DSP paperwork such as logs, activity schedules, inventory, progress notes, and behavior reports.
  5. Maintain cleanliness through supervision of chores, cleaning, cooking, and laundry.
  6. Participate in interdisciplinary team meetings and provide relevant information for evaluation.
  7. Administer medications following CHESCO Services guidelines.
  8. Prepare copies of direct care paperwork (Third Shift).
  9. Attend in-service training and staff meetings.
  10. Transport consumers as needed.
  11. Conduct monthly fire drills and disaster drills as required.
  12. Provide coverage for other shifts and community programs as needed.
  13. Buy groceries and supplies weekly or as needed.
  14. Handle clothing purchases (Second Shift; delegated by House Manager).
  15. Prepare food and supply inventory (Third Shift).
  16. Maintain Service Provision Log (Third Shift).
  17. Prepare chore list (Third Shift).
  18. Perform filing and other duties as assigned.
