Job Responsibilities
  • Provide a safe, clean, and home-like environment; inform Program Manager of physical plant needs.
  • Report serious incidents or rights violations according to policies.
  • Develop ADL and ILS skills related to residents.
  • Maintain knowledge of residents' medical, psychological, cognitive, and social needs; ensure appropriate programming.
  • Accurately record and report residents' development, progress, and related information.
  • Maintain medication records, order medications, and schedule appointments.
  • Review medical records and communicate with medical staff about appointments and follow-ups.
  • Participate in treatment meetings and residents’ reviews.
  • Provide habilitation services as per individual plans.
  • Transport residents to appointments and activities.
  • Assist residents in skill development and daily living activities.
  • Report deficiencies, abuse, or safety issues promptly.
  • Meet nutritional needs and assist with feeding as needed.
  • Organize and supervise meals; handle funds for residents' purchases.
  • Escort residents to medical appointments; complete relevant documentation.
  • Communicate effectively with staff and maintain medical appointment follow-up.
  • Assist with household cleaning and facility maintenance.
  • Participate in medication certification and other assigned duties.
  • Maintain cultural and socioeconomic sensitivity.
Qualifications
  • High School Diploma or GED required; Associate’s degree preferred.
  • Minimum one year experience in Human Services.
  • Excellent communication skills; computer proficiency.
  • Experience with medical care and interacting with healthcare professionals.
  • Must pass AMAP certification, SCIP R, CPR, and First Aid.
  • Valid driver’s license with ability to operate large vehicle (wheelchair or van).
