Qualifications

• High School Diploma or equivalent is required.
• Experience performing related job duties preferred.
• Valid CPR/FA certification required (provided upon hire as needed).
• Valid SCIP certification required (OPWDD Only – provided upon hire as needed).
• Valid TRUST certification required (OMH Only – must be obtained within 120 days of hire).
• Valid AMAP certification required (OPWDD Only – must be obtained within 120 days of hire if needed).
• Valid G-Tube and Insulin certifications required for specific OPWDD programs (must be obtained within 120 days of hire if needed).
• Functional proficiency in spoken and written English.
• Proficient writing skills with grasp of basic grammar and spelling.
• 18 years of age or older; valid driver’s license required.

Responsibilities

• Help individuals achieve personal life goals and daily living skills.
• Advocate and support health and safety for those we support.
• Teach new life skills and support decision-making processes.
• Document services, notes, and information accurately.
• Transport clients to locations outside the home using a motor vehicle.
• Collaborate with care teams to develop and execute service plans.
• Promote community involvement and meaningful activities.
• Provide positive behavior support as needed.
• Maintain confidentiality and comply with safety standards.
